WHO WE ARE

Constellation Mental Health Services is a team of psychologists, psychometrists, therapists, allied health professionals, and administrative staff offering virtual therapy, psychological assessments, wilderness therapy, and trauma treatment. We may also collaborate with consultants and agencies who, in the course of their work, may have limited access to personal information. Every member of our team is bound by confidentiality and committed to protecting your privacy.

WHAT IS PERSONAL INFORMATION?

Personal information includes any data that can identify an individual, such as:

  • Personal characteristics (e.g., name, address, date of birth, gender, phone number)

  • Health details (e.g., psychological history, medical conditions, services received)

  • Opinions or views (e.g., beliefs, preferences, or subjective statements)

 

OUR COMMITMENT TO PRIVACY

We are committed to handling your personal information responsibly and transparently. We collect, use, and disclose personal information only as necessary to provide our services, and always with your informed consent except in rare legally mandated circumstances.

 

CONSENT

You have the right to decide how your personal health information is collected, used, and shared. In most situations, we will obtain explicit consent before gathering personal information. In exceptional cases (e.g., relaying a message through a family member), information may be collected without formal consent when it is reasonable to assume permission would be granted.

 

WHY WE COLLECT PERSONAL INFORMATION

We collect personal information in order to:

  • Assess psychological, developmental, and health needs

  • Offer therapeutic recommendations and interventions

  • Conduct psychological and psychoeducational assessments

  • Provide treatment and support

  • Report on impairments, prognosis, and eligibility for benefits (e.g., disability claims)

  • Share necessary documentation with authorized individuals or institutions

  • Process payments and manage billing

  • Conduct quality assurance and supervise staff

  • Comply with audits or regulatory standards

  • Support due diligence processes if Constellation Mental Health Services undergoes changes in ownership (with confidentiality protections in place)

 

DISCLOSURE OF INFORMATION

We only share your personal information with authorized parties and treatment providers, and only with your consent. Exceptions to confidentiality are limited to the following situations:

  • If there is a clear risk of harm to you or others

  • If there is suspected or disclosed child abuse

  • If there is a report of sexual abuse by a regulated health professional

In these cases, disclosure is required by law.

 

PROTECTING YOUR INFORMATION

We take extensive measures to protect your information, including:

  • Secure storage of all records

  • Use of password-protected, encrypted digital systems

  • No storage of personal data on local devices

  • Staff trained in confidentiality and privacy protocols

  • Consent for services can be withdrawn at any time

 

RETENTION AND DESTRUCTION

Client records are retained for at least 10 years after the last contact for adults, and until a minor reaches the age of 31. After this period, files are destroyed securely and permanently.

 

ACCESS AND CORRECTION

You have the right to access your records, request corrections, and add a statement of disagreement if a correction is not made. Requests must be submitted in writing and may be subject to identity verification and a nominal processing fee.

 

SMS CONSENT

If you choose to receive SMS reminders or communication from us, your consent is used strictly for that purpose. SMS consent is not shared with third parties or affiliates for marketing purposes.

 

CHANGES TO THIS POLICY

This policy may be updated to reflect changes in practice, technology, or regulation. Updates will be posted on our website, with the most recent revision date indicated.
